Calculate your cardiac risk with the American Heart Association’s PREVENT equations, the ASCVD risk calculator used in the 2026 US cholesterol guideline. It runs in your browser: nothing you enter is saved or sent.
This calculator shows the estimate the current ACC/AHA guideline uses: your chance of a heart attack or stroke (atherosclerotic cardiovascular disease, or ASCVD) over the next 10 years, plus total cardiovascular risk including heart failure. It is a learning tool to bring to a conversation with your clinician. Cardio Res-Q does not calculate a PREVENT score as part of its assessments.
You need numbers from a recent check-up: total and HDL cholesterol, your top blood pressure number and your eGFR, a kidney measure printed on most routine blood panels. It is for adults aged 30 to 79 who have never had a heart attack, stroke or heart failure.
The 2026 ACC/AHA cholesterol guideline sorts 10-year PREVENT risk into four bands. The calculator applies them to your 10-year heart attack and stroke estimate.
| 10-year risk | Band | What it usually means |
|---|---|---|
| Under 3% | Low | Healthy habits are the main focus. |
| 3% to under 5% | Borderline | Risk enhancers can shift the picture. |
| 5% to under 10% | Intermediate | Extra information often helps the treatment discussion. |
| 10% or higher | High | A treatment discussion with your clinician is warranted. |
PREVENT gives lower numbers than the older Pooled Cohort Equations for most people, so do not compare a PREVENT result with the old 7.5% cut point. See PREVENT vs. the Pooled Cohort Equations.
This page uses the base PREVENT model. The full model can also add HbA1c, a urine albumin test and a zip-code measure, which change the estimate a little for most people. For more on how the equations work, read the PREVENT risk calculator explained. If a borderline or intermediate number leaves you unsure, see what to do next, or look at what the Cardio Res-Q assessments measure.
This calculator is an educational estimate and is not medical advice. It is not for people who already have cardiovascular disease, and it does not replace a conversation with a clinician who knows your history. Do not start, stop or change a medication based on it. If you have chest pain, shortness of breath or other symptoms of a heart attack or stroke, call 911.
Cardio Res-Q brings advanced labs, arterial imaging and Dr. Varano’s clinical evaluation into one prevention plan. Assessments start at $775.
